Documents Requirement for Renal Donor in India
List of Documents ( for the related donor)
1. Donor and recipient should carry their medical Visas.
2. The passport photocopy of the donor, recipient and Donor's near relative {spouse- if married, parents -if unmarried, Siblings -if unmarried and parents not available} needs to be brought.
3. Passport size photographs for the donor, Recipient (15 photographs each) and Donor's near relative(2 photographs each) should be carried.
4. The patient should bring NOC from the embassy with all the documents attested.
5. Patient must have a family photo depicting the donor, recipient, and the donor's near relatives with all the family members with him or her.
6. Birth certificate/School Certificate/any government ID proof showing both the parent’s names to establish the relationship of the Recipient and Donor
7. Donor ought to bring a marriage certificate (if married).
8. Patient must carry all the government photo ID proofs with an address for identity and relationship for the Donor, Recipient, Donor's near relative and Family Tree.
9. Donor and recipient, both should bring bank account statement and ITR for three financial years
10. Donor's near relatives should be present before the Authorization committee meeting.
11. Every document should be translated into English and attested by the translator.
12. Patients need to bring medical papers including all reports from their native country.
List of Documents required (for Unrelated Donor)
1. Medical visa for the donor, recipient and the attendants is required.
2. Photocopy of the passport for donor, recipient and donor's close relatives (spouse- if married, parents -if unmarried, siblings -if unmarried and parents not available).
3. Passport size photographs; recipient and donor (15 copies each) and donor's close member (2 copies each)
4. The patient should carry a NOC from the embassy with all set of documents attested.
5. Also, the patient need carry a family photograph depicting donor, recipient, Donor's near relative with all the family members.
6. Birth certificate/School Certificate/any government ID proof (showing parents name) to establish the relationship of the donor and recipient.
7. The donor should carry his her marriage certificate (if married).
8. Government photo ID proofs with the address for identity and relationship of the Donor, Recipient and Donor's near relative
9. The Proof of unfit donors in the family should be submitted. Carry their Blood group reports or treatment paper as well.
10. The area head should attest the Recipient's Family Tree. Also, he or she should be ready with an attested photo of the donor as well as the recipient.
11. Donor and recipient are expected to carry ITR and bank account statement for the last three financial years.
12. Donor's relative should be present before the Authorization committee meeting.
13. Carry all the Medical paper from the country.
14. Bring All the documents( translated in English) and get it duly attested by the translator.
15. All the affidavits should contain photographs, and the magistrate should attest it.
